Enhanced Access
Primary Care Networks have been providing Enhanced Access services, with the goal to provide patients with more access to a variety of primary care services. This service is something provided by all GP practices in Buckinghamshire.
This means you may be offered an appointment outside of the core hours of the surgery such as on a Saturday or before or after traditional opening hours. Your registered practice can confirm when these Enhanced Access appointments are available.
The types of appointments provided are Diabetic Checks, Blood Tests, Vaccinations and other routine appointments.

Care Homes
Our Primary Care Network (PCN) supports patients in care homes. Clinical and non-clinical staff from both the PCN and the surgeries visit and support the care homes. This enables patients to receive the best care and provides holistic treatment to all residents.
Our PCN covers the following care homes:
Haddenham Medical Centre – Bartlett’s Residential Care Home, Stone House Nursing Home and Haddenham Park.
Unity Health- Hempton Field Care Home
Cross Keys Practice – Icknield court and Cherry Tree Nursing Home
Medication Reviews
Our Pharmacy Team provides comprehensive medication reviews that take an in-depth look at each patient’s current prescriptions, ensuring their treatments are optimised and that they are fully informed about their medications.

Home Visiting Service
The PCN provides a paramedic to all surgeries in the PCN who will visit their housebound patients at home. Home visits need to be requested before 10am to ensure the best chance of us being able to attend. Home visits are only for housebound patients and for those too ill to attend the surgery.
Home visits are not for those who do not have transport. It is clinically better for patients to be seen at the surgery where they have better facilities, diagnostic aids, and lighting. The clinician can typically see four patients in the time it takes to do a single home visit.

The Joy App
Joy is a tool used to refer patients to services within the community. Our Social Prescribers may also use this tool to manage their caseload.
